Senior Total Health Network was founded by Nancy Rodriguez, RN, BSN, PHN, MBA who has over 20 years of experience working in the Healthcare Industry and has extensive knowledge working with older adults and their families.
Nancy has a background working with the senior population through nursing roles in Emergency Nursing, Pain Management, Case Management, Discharge Planning, Skilled Care/Rehab, and Long-Term Care. Through these roles, Nancy developed strong skills crucial in supporting elderly patients and their families through the challenges of aging.
Education:
Nancy earned an Associate in Science, Nursing degree from Long Beach City College, a Bachelor of Science, Nursing degree from Western Governors University, Master of Business Administration degree from Colorado Technical University. Nancy holds a Registered Nurse license and a Public Health Nurse license in the state of California.
Senior Total Health Network came about after years of working in the healthcare system. Nancy encountered many older adults who experienced debilitating illnesses and Nancy saw firsthand what kind of support the healthcare system was able to provide and what services were lacking. When preparing patients for discharge, Nancy performed assessments to create safe discharge plans; however, Nancy found that many older adults needed ongoing support even after they were discharged from the hospital. That is when Nancy set out to create a company that would help older adults and their families navigate the complex healthcare system while trying to help older adults achieve Total Health that would help them prevent hospital readmissions and live safer and happier lives.
“Total Health” is a term that refers to a state of overall well-being, encompassing physical, mental, and social health. It emphasizes the idea that good health is not just the absence of disease or illness, but rather the presence of positive health factors, such as healthy lifestyle habits, emotional balance, social support, and a sense of purpose and fulfillment in life.
In essence, Total Health involves taking a holistic approach to health and wellness, recognizing the interconnectedness of various aspects of our lives, and striving to achieve optimal health and wellness in all areas. It involves maintaining a healthy lifestyle, including regular exercise, a nutritious diet, sufficient sleep, engaging activities in everyday life, and stress management. It also involves prioritizing mental and emotional well-being, developing positive relationships, and finding meaningful ways to contribute to society. What makes a Geriatric Care Manager stand apart from the others is the commitment to stay focused on the needs of the client and family. Others may offer services for less, but what you receive cannot help but be influenced by the payment structure.
Hospital- or insurance-based case managers are employees of the health care system. They are good professionals. Their focus, however, is to address the issues of patients within the institution and to make efficient use of services for their organization.
We are based in the community and work with aging adults wherever they live. We provide continuity of care across all settings. We include the family and focus on improving quality of life for the older adult and their grown children.
Senior advisors are paid referral fees to suggest elder care facilities. As a general rule, they have a more limited sphere of knowledge and do not hold a professional degree needed to give them expertise in the 8 key areas of elder care. Only those facilities willing to pay a finder’s fee will be included in their list of suggestions. There may be a more appropriate option, but it will not be mentioned if the facility does not pay for referrals.
Because we are paid directly by you, we are independent and objective. Our focus is 100% on the needs of your family and your aging loved ones. We apply our considerable skill and years of training to your unique situation.
